4.2 Which computer programs/applications have been used for emission/risk evaluation?  
4.3 Are revamps or additions to existing plant included in the scope of design?  
4.4 If so, have all possible consequences and effects on existing equipment and piping been evaluated?  
  Are inspection reports on existing units made available?  
  Has plant safety been compromised in any way by the additions?  
  If so, what steps have been taken to retain the original safety integrity of the plant?  
  Has client/licensor(s) been involved in the revamp detail and the additional changes required to achieve a safe plant?  
4.5 Have process safeguarding flow schemes been developed?  
  If so, who is responsible for these schemes? (client, licensor or Company).  
4.6 Has the Company engineering guide BN-EG-UE2 been followed to establish mechanical design conditions?  
  Has client(licensor(s) received a copy of this document?  
4.7 Has a study been made regarding ergonomy?

  If yes, what were the conclusions and are they implemented in the design?

4.9 Have hazard and operability (HAZOP) studies been performed, or are any planned?

Hazard and Operability Issues  
  If no hazard and operability study or safety review is planned or has been performed, the following questions should be answered:  
  Are there any hazards in the plant associated with:  
5.1 High or Low Temperatures  
  What are the maximum/minimum temperatures that will be experienced in normal/abnormal operation and what, if any, special design margins or procedures have been used to allow for such temperatures?  
5.2 High or Low Pressures  
  What are the maximum/minimum pressures that will be experienced in normal/abnormal operation and what, if any, special design margins or procedures have been used to allow for such pressures?  
5.3 Chemical Reaction  
  Have all chemical reactions in the plant been studied from a safety standpoint, especially with respect to possible temperature runaway situations?  
  What special precautions have been adopted to cope with such operational excursions?  
5.4 Chemical Reactive Characteristics  
  Do any feedstocks, intermediate products, final products, catalysts, chemicals, solvents, vapor/ liquid/solid effluents or dusts used/produced in the plant display any hazardous characteristics such as being pyrophoric, exothermic, unstable, explosive, oxidizing, spontaneously ignitable?  
  If so, what procedures have been incorporated to allow for these characteristics?  
5.5 Toxic Substances  
  Have all toxic substances, including those that may form during abnormal operation, been identified? Is a complete material safety data sheet available?  
  What methods of containment during normal operation have been incorporated in the plant design and what special procedures, if any, have been developed to counteract accidental toxic spillage or vapor release?  
5.6 Corrosive Substances  
  Does the plant handle any unusually corrosive substances?  
  What special materials have been specified for such corrosive duty and what data is available on the successful use of such materials in the particular environment?  
  Were external corrosive effects as well as internal effects considered when materials were selected?  
  Was client/licensor(s) involved or informed of the design decisions in these particular areas?  
5.7 Radiation Emissions  
  Are there anyradioactive emissions from the plant?  
  If so, what design standards and codes of practice have been followed to counteract such emissions?  
5.8 Inventories  
  What steps have been taken to minimize or remove hazardous inventory within the on-plot plant?  
  What is the design basis for the operating holdup in tower sumps, overhead receivers and separators?  
  What hazardous materials are stored on-plot in day/surge drums e.g. fuel oil, solvents, feed, intermediate and final products?  
  Have all possibilities to remove this storage off-plot been investigated?  
  Has client been informed of the possible hazards associated with the on-plot inventories?  
5.9 Startup Procedures  
  During the development of the startup procedures, were all possible procedural deviations/errors/ failures considered e.g. fresh catalyst charge requiring abnormal operating conditions during first days of life?  
  What appropriate steps were taken and are these detailed in the operating manual?  
  Are the maximum operating parameters attributable to the above, accounted for in the equipment design?  
  What input did client/licensor(s) have in formulating the operating possibilities that might be encountered during a normal startup?  
5.10 Shutdown Procedures  
  Do the normal shutdown procedures allow for steady throughput reduction to zero followed by, depending on short or long term shutdown, detail of all safety-related aspects such as inert gas blanketing, purging, fired heater cool down, pump draining inventory movement off-plot?  
  What involvement/input did the client/licensor(s) have?  
5.11 Emergency Shutdown Procedures  
  Is the control system designed to automatically shutdown the plant to a safe situation in the event of, for instance, a power failure or a temperature runaway in a reactor?  
  Do the procedures detail in order the most important actions to be taken immediately by the plant operators in the event of an emergency shutdown?  
  Has client/licensor(s) been involved in the development of the shutdown system and procedures?  
5.12 Are there any societal hazards associated with the plant such as release of toxic gases or wastes, fire or explosion which could result from:  
  a. maloperation;  
  b. malfunction;  
  c. failure of equipment;  
  d. emergency procedures.  
  If so, have all possible consequences of such hazards been considered and have all steps been taken, in terms of design and emergency actions, to minimize the effects of such a societal incursion?  
5.13 Are there any other areas in the plant design and operating procedures that are potential sources of hazard and that are not included in the preceding questions?  
  If so, provide a list of such items with the appropriate steps, if any, that have been taken to reduce the hazard to a level acceptable to Company/client/licensor(s)/local authorities.
